For fifteen years the industry digitized the residue: paper JSAs became PDF JSAs, binders became portals, and the scoreboard was adoption. But a completed form is not a safe decision — it is a record that a decision was supposedly made. That gap, between the record and the decision, is where people still get hurt, and it is why so many well-funded safety programs stall the moment the forms go live. The unit of safety is not the form. It is the conversation that produces it.

One hundred percent form completion tells you the paperwork happened. It tells you nothing about whether the crew named the real hazard, whether the new sub understood the plan, or whether the control was the right one. Read the levels as a ladder, not a menu — you cannot skip. Predictive Safety at Level 5 is arithmetic performed on the conversations captured at Levels 2 through 4, so an organization that has not fixed capture and adaptation cannot leap to prediction. The most important line in the model sits between Level 2 and Level 3: Levels 1 and 2 improve the record; Level 3 and above improve the decision. That leap — from documentation to decision — is where incident numbers change.

Run it as a working session, not an audit. Put the model in front of the people who see the field — an operations lead, a field supervisor, a safety rep, and if you can, a crew member — and score honestly. Most organizations find they are one level below where they assumed, and the gap is always in the same place: they improved the record and never improved the conversation.

Climb One Rung at a Time

If you are at Level 1, the next move is faithful capture, not predictive analytics. Prediction built on thin data is confident and wrong — and the most costly mistake is buying Level 5 analytics on top of Level 1 data.

RULE 2 · MEASURE THE CONVERSATION

Not Just the Completion

Track whether briefs get updated when conditions change, whether crews report near misses, and whether missing-hazard prompts get acted on. Those are Conversational Safety metrics. Completion rate is a Level 1 metric.
