Previous analyses can provide a useful starting point. The problem begins when familiar wording replaces a fresh look at the equipment, personnel, work area, surrounding operations, and current conditions.

Making Job Steps Too Broad

Naming the Hazard Without Describing the Exposure

“Be aware,” “use PPE,” or “follow procedure” can leave too much unresolved when the form does not make clear what action will address the hazard.

Treating Sign-Off as the End of the Analysis

Signing the JSA records the plan at a point in time. The job can continue changing after that point, which means the analysis may need to change with it.

When Should a JSA Be Reviewed or Updated?

A JSA can accurately describe the job during the morning briefing and stop matching the work later in the shift.

  • A close call exposes a weakness in the original plan
